By maximizing the potential of Medicare Advantage, we help seniors access comprehensive care and make informed decisions about their health. Kitzo Healthcare Partners collaborates with Medicare Advantage payors and primary care providers to focus on increasing patient NPS, reducing ED visits, reducing hospital sick days, managing chronic illnesses, and optimizing utilization. In this effort we also looking to increase attention to HEDIS measures and CMS Stars ratings.

Equity in access to healthcare is a pressing concern that we actively address. With up to 80% of a patient’s health outcomes being related to social factors (Social Determinants of Health) and not clinically driven, we recognize that wraparound service navigation is critical for low-income populations and seek to assist providers find solutions for these needs. These “social barriers” create access issues with further exacerbates health disparities. That's why we prioritize the social determinants of health (SDOH) in our approach. By understanding and addressing factors such as housing, nutrition, education, and employment, we strive to improve health outcomes and promote equity for all individuals, regardless of their socioeconomic status.
